balloon tamponade vs esophageal stent in controlling acute refractory variceal bleeding: A multicenter RCT. Management of malignant stricture of the esophagogastric junction with a newly designed self-expanding metal stent with an antireflux mechanism. Superiority of anti-reflux stent compared with conventional stents in the palliative management of patients with cancer of the lower esophagus and esophago-gastric junction: results of a randomized clinical trial. feasibility and safety of a new, implantable reflux control system to prevent gastroesophageal reflux in patients with stents placed through the lower esophageal sphincter (with video). Gastrointest Endosc 2012;75:174-178. 5) Zehetner J, Shamiyeh A, Wayand W, Hubmann R. Results of a new method to stop acute bleeding from esophageal varices: implantation of a self-expanding stent. We read with interest the letter from Dai and coworkers related to the potential reflux-related complications of esophageal stents. However, the complications the authors described were related to long-term esopha-geal stents used to treat benign or malign strictures of the esophagus, which is not the case. In fact, the stents used to control acute esophageal variceal bleeding are specifically designed for this severe complication and are removable stents, which can remain in place from 6 to 14 days. Moreover, they were also used to treat esophageal tears caused by a previous balloon tamponade. As stated by the authors, all patients developing acute variceal bleeding are placed in a slightly upright position to avoid aspiration of the gastric content, which, unfortunately , occurred in one patient in the stent group. However, this adverse event could be attributed to the presence of hematemesis, upper endoscopy, placement of the stent, etc. As mentioned in the article, aspiration was more frequent in the group of patients treated by esophageal balloon tam-ponade than in those receiving esophageal metal stents. A self-expanding metal stent for complicated variceal hemorrhage: experience at a single center. removable covered self-expanding metal stent for the management of Sengstaken-Blakemore tube-induced esophageal tear and vari-ceal hemorrhage. Peiffer et al. recently reported (1) an association between the host interferon lambda 4 (INFL4) single-nucleotide polymorphism (SNP), rs12979860, and the NS5A resistance-associated variant (RAV) Y93H in hepatitis C virus (HCV) genotype 1b (HCVg1b). This observation is intriguing because it directly links innate immunity to HCV viral drug resistance for the first time. A small cohort of (51) HCV genotype 3 (HCVg3) patients was included in the analysis; this subgroup analysis was underpowered and no association was observed in HCVg3. …

Resistance-Associated NS5A Variants of Hepatitis C Virus Are Susceptible to Interferon-Based Therapy
BACKGROUND & AIMS The presence of resistance-associated variants (RAVs) of hepatitis C virus (HCV) attenuates the efficacy of direct acting antivirals (DAAs). The objective of this study was to characterize the susceptibility of RAVs to interferon-based therapy. METHODS Direct and deep sequencing were performed to detect Y93H RAV in the NS5A region. Twenty nine genotype 1b patients with detec...
متن کاملNatural prevalence of resistance-associated variants in hepatitis C virus NS5A in genotype 3a-infected people who inject drugs in Germany.
BACKGROUND People who inject drugs (PWID) are the most important risk group for incident Hepatitis C virus (HCV) infection. In PWID in Europe HCV genotype 3a is highly prevalent. Unfortunately, many of the recently developed directly acting antiviral drugs against HCV (DAAs) are suboptimal for treatment of this genotype. Detection of resistance-associated variants (RAV) in genotype 3a may help ...
متن کاملCharacterization of Naturally Occurring NS5A and NS5B Polymorphisms in Patients Infected with HCV Genotype 3a Treated with Direct-Acting Antiviral Agents
Hepatitis C virus (HCV) genotype (GT)3 is associated with increased risk of steatosis, development of cirrhosis and hepatocellular carcinoma. Limited data are available regarding genetic variability and use of direct-acting antiviral agents in these patients. non-structural protein 5A (NS5A) and non-structural protein 5B (NS5B) sequencing was performed on 45 HCV GT3-infected Italian patients su...
متن کاملInterferon-lambda genotype and low serum low-density lipoprotein cholesterol levels in patients with chronic hepatitis C infection.
UNLABELLED Recently, genetic polymorphisms occurring in the interferon (IFN)-lambda gene region were associated with response to IFN-based treatment of hepatitis C infection. Both infection with the hepatitis C virus and IFN therapy are associated with decreased serum cholesterol and high cholesterol has been associated with increased likelihood to respond to IFN. We sought to determine if the ...
